Sam Madison in his career played for two NFL teams. He played for Miami Dolphins (1997–2005) and New York Giants (2006–2008).

Madison was taken in the second round (44th overall) of the 1997 NFL Draft by the Miami Dolphins. Madison was the sixth cornerback taken in the 1997 NFL Draft.

From 1999 through 2002, Madison was named to the Pro Bowl four times in a row. The Dolphins cut him on March 1, 2006.

He started 127 of 138 games for the Dolphins, racking up 353 tackles, a sack, 31 interceptions, and two touchdowns in the process.

Sam Madison played alongside fellow cornerback Patrick Surtain for the majority of his Miami Dolphins tenure.

Madison and Surtain were one of the most productive cornerback duos in NFL history, combining for 697 tackles, 7.5 sacks, 60 interceptions, and two touchdowns during their career together.

On March 10, 2006, Madison signed a four-year contract with the New York Giants. With the Giants, he won a Super Bowl ring in Super Bowl XLII against the New England Patriots.

Madison only played in seven games for the Giants in 2008, but he had eight tackles and an interception.

On December 30, he was placed on season-ending injured reserve with a broken ankle. On February 9, 2009, he was released.
