He made his professional debut in 2012 with the Low-A Williamsport Crosscutters and also pitched for the Single-A Lakewood BlueClaws, posting a combined 7-3 record and 2.50 ERA.

In 26 games with the High-A Clearwater Threshers in 2013, he has a 12-7 record with a 3.83 ERA. He pitched for the Reading Fightin Phils in Double-A in 2014, posting a 10-6 record with a 4.21 ERA and 86 strikeouts.

In 30 appearances in 2015, he split his time between Reading and Clearwater, posting a 2-1 record and 3.52 ERA.

Milner went 5-4 with a 2.49 ERA in 49 games with the Triple-A Lehigh Valley IronPigs and Reading Royals in 2016. Milner was drafted in the Rule 5 draft by the Cleveland Indians in 2016.

Milner was returned to the Phillies by the Indians on March 24, 2017. Milner began 2017 with the Lehigh Valley IronPigs before being promoted to the Philadelphia Phillies on June 20.

On June 24, he made his MLB debut against the Arizona Diamondbacks, tossing 1.0 inning and conceding one run. Milner pitched 37 innings in 2017 and has a 2.01 ERA.

Milner was designated for assignment on July 8 after posting a 7.71 ERA in 10 appearances in 2018.
