Cousins was promoted to the major leagues for the first time on June 21, 2021, when he was named to the 40-man roster. Prior to his call-up, Cousins had a 2.55 ERA in 16 games between Biloxi and Nashville.

That day, he made his MLB debut against the Arizona Diamondbacks, tossing two shutout innings of relief. He struck out Diamondbacks catcher Stephen Vogt for his first career strikeout in the game, one of five on the night.

Cousins earned his first career victory on July 7 against the New York Mets, tossing a scoreless inning in relief. Until his 18th pitching appearance, Cousins had not allowed an earned run.
