Cisnero signed as an international free agent with the Houston Astros on December 6, 2007, and was added to their 40-man roster on November 20, 2012. On April 22, 2013, he made his big league debut.

He made 33 bullpen appearances for the Astros over portions of two seasons, collecting a 4.66 ERA with 46 strikeouts in 48+13 innings. On November 3, 2014, he was released from his contract and became a free agent.

Cisnero signed a minor league agreement with the Cincinnati Reds on November 18, 2014. Due to Tommy John surgery, Cisnero missed the 2015 season. On November 6, 2015, he chose free agency.

Cisnero signed a minor league contract with the Arizona Diamondbacks on December 21, 2015. After failing to make the squad in Spring Training, he was released on March 31, 2016.

Cisnero signed with the Sultanes de Monterrey of the Mexican Baseball League on May 3, 2016. On May 30, 2016, he was released.

He joined the Can-Am League’s New Jersey Jackals on July 12, 2016. Before being released on July 25, he appeared in five games out of the bullpen and had a 3.86 ERA.

Cisnero was signed to a minor-league deal with an invitation to spring training by the Detroit Tigers on November 9, 2018.

The Toledo Mud Hens were his first stop in the 2019 season. The Tigers selected his contract on June 22, 2019.

The next day, he made his big league debut for the Tigers, marking his first appearance in over five years. In 35 games, he pitched to a 4.33 ERA while striking out 40 batters in 35+13 innings.

Cisnero pitched in 29 games for the 2020 Detroit Tigers, posting a 3–3 record with a 3.03 ERA and 34 strikeouts in 29+23 innings pitched.
