Jake was drafted by the Tampa Bay Rays in the 2004 MLB draft. He was selected in the 5th round of the draft and made his minor league debut that season playing for the Princeton Devil Rays and the Hudson Valley Renegades.

During the 2006 baseball season, he was made to spend it with the Southwest Michigan Devil Rays and split his 2007 season playing for the Vero Beach Devil Rays and the Montgomery Biscuits. He signed a two-year contract with the San Francisco Giants in February 2021. The contract was worth $7 million with a club option for the 2023 baseball season. In July of that same year, he was named National League Reliever of the Month for July 2021.
