On June 10, Cain was called up to the Triple-A Nashville Sounds to take the position of outfielder Hernán Iribarren, who had been recalled by Milwaukee after starting the 2008 campaign in Brevard County.

On July 16, 2010, when pitcher Doug Davis was placed on the disabled list, Cain was promoted to the Brewers. That evening, he pinch-hit against the Atlanta Braves to make his major league debut.

In his first at-bat, Matt Diaz of the Braves’ left-field team made a diving catch to deny him a hit. He got his first major league hit two days later during his second at-bat in the same series.

He subsequently took over for Carlos Gómez as the center fielder for the remainder of the game, going 2 for 2 with two singles. Cain later played in 43 games for the Brewers.

He was exchanged for Zack Greinke and Yuniesky Betancourt by the Kansas City Royals on December 18, 2010, along with Alcides Escobar, Jeremy Jeffress, and Jake Odorizzi.
